Have you ever looked at your lab results, breathed a sigh of relief because everything came back normal, and then wondered why you still felt exactly the same way. Maybe you're exhausted by the middle of the afternoon, even after a full night's sleep. Your hair doesn't seem full as it used to, the scale keeps creeping up even though your eating habits haven't really changed. You're struggling with constipation, your skin feels drier than it used to, and sometimes you find yourself reaching for a sweater while everyone else seems perfectly comfortable. Then someone tells you it's probably just menopause, or maybe you've been told your thyroid is normal. So you begin asking yourself, then why do I feel this way? If you've ever wondered whether your thyroid could be playing a role in how you feel, or whether there might be more to the story than one normal lab test, this episode is for you. Today we're going to talk about one of the most misunderstood topics in women's health. Not because thyroid disease is uncommon, but because understanding the thyroid isn't as simple as checking one lab value or blaming every symptom on hormones. We'll explore what thyroid actually does, why thyroid problems often become more noticeable during perimenopause and menopause, what a TSH test really tells us and what it does not tell us, and why your symptoms deserve to be evaluated in context of your entire health picture. Because your thyroid matters, but it may not be the whole story.

Why We Look For Patterns

This podcast is for women who want to understand why they feel the way they do so they can make informed decisions about their health and create a longer, healthier, more vibrant life. One of the things I've learned after caring for women for more than 20 years is that the body is almost always giving us clues. The challenge isn't that our bodies aren't communicating, it's that no one has ever taught us how to listen. Around here, we don't chase symptoms. We look for patterns. We connect the dots between hormones, nutrition, metabolism, sleep, stress, movement, inflammation, and the many systems that are constantly communicating inside your body. Because when you understand the why, you can make better decisions about the what. Today's episode is one that I have wanted to do for a long time because I hear some version of this question almost every week in my practice. A woman in their late 40s comes in to see me because she was convinced something was wrong with her thyroid. She told me I don't recognize myself anymore. She was exhausted, gaining weight, losing hair, and couldn't concentrate at work. Her first question was exactly the one I hear all the time. Can you just check my thyroid? Over the next hour, we talked about her symptoms, reviewed her medical history, looked at her lab, and started putting together the puzzle. That's really what today's episode is about. The challenge is that every one of those symptoms can have multiple causes. They may be related to the thyroid function, they may also be related to perimenopause. They could be related to iron deficiency, poor sleep, chronic stress, nutrient deficiencies, medications, depression, autoimmune disease, or several things happening at the same time. That's why I wanted to start today's conversation with one important reminder. This podcast is designed to educate you and help you better understand your body. It is not intended to diagnose medical conditions or replace the relationship you have with your health care provider. My goal is to help you ask better questions, understand your lab results more confidently, and recognize when it may be appropriate to have a deeper conversation with your clinician. I also want to clear up another common misconception before we go any further. This episode is not about convincing you that everyone has thyroid disease. The thyroid has become a popular topic on social media, and unfortunately, there is a lot of misinformation. Some people believe every symptom means they have hypothyroidism. Others believe that if their TSH is normal, there's absolutely no way their thyroid could be contributing to how they feel. The truth, as it often is in medicine, lies somewhere in the middle. The TSH is an excellent screening tool and has helped millions of people receive appropriate diagnosis and treatment. At the same time, no single lab value tells us everything about a person's health. As clinicians, we don't treat laboratory numbers. We care for people. That means we consider symptoms, medical history, family history, physical exam, medications, lifestyle, and when appropriate, additional laboratory testing. Medicine is rarely about finding one magic answer. It's about putting together a puzzle. Every symptom is another piece of that puzzle. And today we're going to explore where the thyroid fits into that puzzle and why understanding its role can be an important part of protecting not only how you feel today, but also your long-term health span. So

What The Thyroid Actually Does

let's start with a simple question. What exactly is the thyroid? And why does this tiny butterfly-shaped gland have such a big influence on how we feel every single day? Think of every cell in your body as a tiny factory. Thyroid hormone doesn't build the factory. It helps determine how efficiently that factory runs. When thyroid hormone levels are too low, it's almost as if every factory slows production. Energy production slows, digestion slows, skin turnover slows, hair growth slows, cholesterol processing slows, even the way your brain processes information can feel slower. That is why a small gland can create symptoms throughout the entire body. The thyroid is a small, butterfly-shaped gland that sits at the front of your neck, just below your Adam's apple. Despite its size, it has an enormous impact on nearly every cell in your body. One of the easiest ways to think about your thyroid is as your body's metabolic regulator. It doesn't simply determine how many calories you burn, it influences how efficiently your cells produce and use energy. Every second of every day, your body is converting nutrients from the food you eat into energy that powers your brain, muscles, heart, digestive system, immune system, and every other organ. Thyroid hormones help regulate that entire process. When your thyroid is functioning well, those systems generally work together efficiently. When thyroid hormone levels are too low or too high, the effects can be felt throughout the body. Think about the symptoms we often associate with thyroid disorder fatigue, weight changes, brain fog, hair thinning, dry skin, constipation, feeling cold when everyone else is comfortable, changes in your menstrual cycle, mood changes, slower recovery from exercise, elevated cholesterol. None of those symptoms seem related at first glance. One affects the hair, another affects your digestion, another affects your brain. But they're all connected by one common thread. Every one of those tissues depends on thyroid hormone to function normally. That's why thyroid disorder can be so confusing. Instead of causing one obvious symptom, they can often create a collection of seemingly unrelated problems that slowly develop over time. And here's something many women don't realize. Those same symptoms are also incredibly common during perimenopause and menopause. Fatigue, brain fog, weight gain, mood changes, sleep disturbances, dry skin, hair changes. If you're thinking, 'Wait a minute...those sound exactly like menopause,' you're right. There is a tremendous overlap. That's one of the reasons thyroid disorders can sometimes be overlooked during midlife, or why women may assume every symptom their experiences simply due to hormonal changes. The reality is that both can exist at the same time. You can absolutely be experiencing hormonal transition of perimenopause while also be developing thyroid conditions. Or your thyroid may be functioning normally and your symptoms are coming from something entirely different. That is why it is so important not to jump to conclusions in either direction. One of the biggest mistakes I see is assuming there's only one explanation for how we're feeling. Health rarely works that way. Our bodies are wonderfully complex. Hormones influence thyroid function. Thyroid function influences metabolism. Sleep influences hormones. Stress affects the immune system. Nutrition supports every one of those processes. Everything is connected. That's why I often tell my patients that our bodies don't experience symptoms in silos. And we shouldn't evaluate them in silos either. When we step back and look at the whole picture instead of focusing on one symptom or one lab result, we often begin to see patterns that weren't obvious before. And that's exactly how we approach health span. Not by asking what's wrong with one organ, but by asking how are all these systems communicating with one another? Because that's where the real story begins.

Why Symptoms Spike After 40

One of the questions I often hear is why did all of this seem to happen after I turned 40? Well, maybe you felt healthy for most of your life. Then, seemingly out of nowhere, you're struggling with fatigue, brain fog, weight gain, dry skin, poor sleep, constipation, or hair loss. You wonder if it's your thyroid, someone else tells you it's menopause, another person tells you it's stress, social media tells you it's your cortisol, and suddenly you're trying to figure it all out which one is to blame. The truth is our bodies don't work that way. There usually isn't one single explanation. As women move through perimenopause and into menopause, nearly every major system in the body is adapting to changing hormone levels. Estrogen begins to fluctuate and eventually declines. Progesterone declines. Sleep often becomes more disrupted. Muscle mass naturally begins to decline if we aren't actively working to preserve it. Insulin resistance becomes more common. Inflammation may increase. Recovery from stress often takes longer. And because all of these systems communicate with one another, it's easy to see why symptoms can start showing up around the same time. This is also why I encourage women not to dismiss everything as just menopause. Yes, menopause is a normal transition, but normal doesn't mean your symptoms should automatically be ignored. Sometimes menopause is the primary driver, and sometimes it simply uncovers something that was already developing beneath the surface. For example, autoimmune thyroid disease, sometimes much more common in women, especially during midlife. Iron deficiency may develop after years of heavy menstrual bleeding. Poor sleep may be contributing to fatigue and brain fog. Insulin resistance can make weight management much more difficult, and chronic stress may leave you feeling exhausted, even if your thyroid is functioning perfectly. This is why it's so important to resist the urge to look for a single culprit. Your body isn't made up of separate departments working independently. It's more like a symphony orchestra. Imagine sitting in the audience listening to an orchestra perform. If the violins are out of tune, you notice it. If the percussion section is rushing the tempo, the music feels different. If the brass section is too loud, it changes the entire performance. Every section influences every other section. Your body works much the same way. The thyroid doesn't function in isolation. Neither do your ovaries or your adrenal glands or your gut or your immune system. They are constantly communicating. That's why integrative medicine, we don't ask which organ is causing this. We ask what conversations are these systems having with another? Because that question often leads us much closer to understanding what's really happening. Now I also want to reassure you about something. Just because your symptoms overlap with thyroid disease doesn't mean you have a thyroid disorder. And just because you're in perimenopause doesn't mean every symptom is caused by changing estrogen levels. The goal isn't to diagnose yourself, the goal is to recognize patterns and have informed conversations with your healthcare provider. That's one of the reasons I believe understanding your body is so empowering. When you understand the questions to ask, you're less likely to feel dismissed, confused, or overwhelmed. You're able to become an active participant in your healthcare instead of feeling like you're simply waiting for someone else to figure it out.

What A TSH Test Means

Let's talk about one of the most common questions I hear. My doctor checked my thyroid and they said it was normal. So why do I feel exhausted? To answer that question, we first need to understand what was actually tested. In most cases, when someone says my thyroid was checked, they're referring to a blood test called a TSH, which stands for thyroid stimulating hormone. Now here's something that surprises many people. TSH isn't actually thyroid hormone. It's a hormone made by the pituitary gland, a small gland located at the base of your brain. Think of your pituitary as a manager of your thyroid. Its job is to monitor how much thyroid hormone is circulating in your body and send signals to the thyroid when adjustments are needed. If your brain senses that the thyroid hormone levels are dropping, it responds by producing more TSH. It's essentially saying, hey, thyroid, we need more hormone, let's get going, get working. On the other hand, if there's already plenty of thyroid hormones circulating, the pituitary eases off and produces less TSH. So rather than measuring the thyroid hormone itself, TSH measures the communication between the brain and the thyroid. It's an incredibly useful test. In fact, most people, the TSH is the first screening test for identifying thyroid dysfunction. And this is important to understand. A normal TSH is reassuring. It tells us in many cases your thyroid is doing exactly what it should be doing. If you're still having symptoms, it also reminds us to think more broadly about what else could be contributing. So if you have a normal TSH, that doesn't automatically mean something was missed. It means your clinician has gathered one very important piece of information. But here's where medicine becomes a little more nuanced. One laboratory test rarely tells the entire story. As clinicians, we don't diagnose people based on a single number. We look at the whole picture, your symptoms, your medical history, your family history, your medications, your physical exam, your age, whether you're pregnant or not, whether you're in perimenopause, whether you have another autoimmune condition, whether your symptoms are new or have they been presenting for years. All those pieces matter. Sometimes, based on that bigger picture, your healthcare provider may decide that additional thyroid testing is appropriate. That might include measuring your free T4, which is one of the circulating thyroid hormones. Sometimes free T3 may also be helpful in certain clinical situations. If autoimmune thyroid disease is suspected, your clinician may also order a thyroid antibodies test, such as thyroid peroxidase antibody, often called a TPO antibody. These additional tests aren't necessarily for everyone. They're ordered based on the individual's history, symptoms, and clinical judgment. That's one of the reasons I encourage women not to order large panels of laboratory tests simply because they saw someone recommended them on social media. More testing isn't always better. Testing should answer a clinical question. As healthcare providers, we're constantly asking ourselves, will this test change what I do next? If the answer is no, then it may not provide meaningful information. The goal isn't to collect more lab results. The goal is to understand what those results actually mean within the context of the person sitting in front of us. And sometimes that's the hardest part of medicine. Not interpreting one lab, but putting all the pieces together. Because even when the thyroid is functioning normally, many other conditions can produce symptoms that look remarkably similar. Iron deficiency, poor sleep, depression, anxiety, vitamin B12 deficiency, insulin resistance, medication side effects, perimenopause, and chronic stress. Even simply not eating enough protein or losing muscle mass over time can leave women feeling tired, weak, and mentally foggy. That's why I often remind my patients that symptoms don't belong to one diagnosis. Fatigue isn't a thyroid diagnosis, hair loss isn't a thyroid diagnosis, brain fog isn't a thyroid diagnosis. They're clues. And our job is to understand what your body may be trying to tell us. Now

Hashimoto’s And Thyroid Antibodies

let's talk about something you may have heard of, but perhaps don't fully understand: Hashimoto's thyroiditis. If you've been diagnosed with hypothyroidism, there's a good chance you've also heard this name, Hashimoto's, is the most common cause of hypothyroidism in the United States, and in many other parts of the world where iodine intake is generally adequate. But here's what's important to understand. Hashimoto's isn't simply a thyroid problem, it's an autoimmune condition. That means your immune system, which is designed to protect you from infection and keep you healthy, begins mistakenly recognizing parts of your thyroid as something it shouldn't be. Instead of protecting the thyroid, the immune system slowly attacks it. Over time, that ongoing inflammation can reduce the thyroid's ability to produce enough thyroid hormone. That's why many people with Hashimoto's eventually develop hypothyroidism. One thing I find fascinating is the autoimmune diseases, including Hashimoto's, occur much more frequently in women than they do in men. In fact, women account for the vast majority of Hashimoto's diagnoses. Researchers are still working to understand exactly why that is, but it's likely related to the combination of genetics, immune system regulation, hormone influences, and the environmental factors. Another interesting aspect of Hashimoto's is that it often develops gradually. Your immune system may begin producing thyroid antibodies years before your thyroid hormone levels actually become abnormal. That doesn't mean that everyone with thyroid antibodies will immediately develop hypothyroidism. Some people never do. Others may develop thyroid dysfunction over time. This is one reason your healthcare provider considers the entire clinical picture, not just one laboratory result. If someone has symptoms suggestive of thyroid disease, a strong family history of autoimmune conditions, or other clinical findings, thyroid antibodies. Tests may sometimes be appropriate. Again, that's a decision made based on your history and your healthcare provider's clinical judgment. It's also worth mentioning that autoimmune disease tends to cluster. Someone with one autoimmune condition may have a higher likelihood of developing another. For example, Hashimoto's is seen more commonly in people with conditions like type 1 diabetes, celiac disease, rheumatoid arthritis, lupus, or vitiligo. That doesn't mean these conditions always occur together. It simply reminds us that the immune system is incredibly complex. Now I want to pause here because I know it's easy to hear all of this information and begin wondering, do I have Hashimoto's? Maybe, and maybe not. Remember what we've talked about throughout today's episode. Symptoms are clues. They're not diagnosed. Fatigue can occur for dozens of different reasons. Hair loss has as many possible causes as fatigue. Brain fog isn't specific to one disease. Constipation isn't unique to thyroid problems. The goal isn't to diagnose yourself. The goal is to understand your body well enough to have a meaningful conversation with your healthcare provider. And if thyroid disease is part of that picture, identifying it early allows you and your clinician to develop an appropriate treatment plan. If it's not your thyroid, then you've still gathered valuable information because you've ruled out one important possibility and can sometimes look for other causes. That's one of the most important lessons I've learned over the years. Sometimes a normal test result is just as helpful as an abnormal one. Every piece of information helps us narrow the possibilities. Medicine isn't about providing one diagnosis, it's about carefully eliminating the diagnoses that doesn't fit until the picture becomes clear.

Nutrients That Support Thyroid Function

By now, you've probably noticed the theme throughout today's episode. Very little in medicine is as simple as finding one cause or taking one supplement. The thyroid is no exception. Like every other organ in your body, the thyroid depends on having the right building blocks to do its job. That doesn't mean everyone needs supplements. It doesn't mean everyone has deficiency. But it does mean that good nutrition plays an important role in supporting healthy thyroid function. Let's talk about a few of these nutrients that come up most often. We'll start with iron because it's one that surprises many women. Iron isn't just important for preventing anemia. Your body also needs adequate iron to produce thyroid hormone and to help that hormone function normally within your cells. Women are particularly vulnerable to iron deficiency because of menstrual blood loss, pregnancy, and in some cases dietary intake. Heaving menstrual periods during the years leading up to menopause can slowly deplete iron stores without a woman realizing it. Sometimes the first signs aren't obvious. You may notice fatigue, poor exercise tolerance, hair shedding, restless legs, feeling cold, or difficulty concentrating. Those symptoms can overlap with thyroid disease, which is one reason iron status is often considered when evaluating someone with persistent fatigue or hair loss. We actually spend an entire future episode talking about iron because it's such an important topic for women. Selenium is a trace mineral that helps support normal thyroid hormone metabolism and also plays a role in protecting thyroid tissue from oxidative stress. You may have heard people recommend eating Brazil nuts because they're naturally rich in selenium. That's true, but it's also a good reminder that more isn't necessarily better. Selenium is one of those nutrients where both deficiency and excess can create problems. That's why I generally recommend getting nutrients from a balanced diet whenever possible and discussing supplementation with your healthcare provider if there's concerns about deficiency. Zinc is another mineral involved in the thyroid hormone production in immune function. It's also important for wound healing, taste and smell, and overall immune health. Fortunately, most people who eat a varied diet often eat adequate zinc, although certain medical conditions or restrictive diets can increase the risk of deficiency. Again, the key message isn't that everyone needs zinc supplement. It's that your thyroid depends on many nutrients working together. Now let's spend a little more time talking about iodine because it's probably the nutrient most closely associated with thyroid health. Your thyroid absolutely needs iodine to produce thyroid hormone. Without enough iodine, the thyroid can't manufacture those hormones efficiently. Historically, iodine deficiency was a major cause of hypothyroidism around the world. Adding iodine to table salt dramatically reduced iodine deficiency in many countries, including the United States. So if iodine is important, wouldn't taking more iodine be better? Actually, no. This is where social media often gets the story wrong. More iodine is not always better. In fact, people with autoimmune thyroid disease, particularly Hashimoto's, excess iodine may actually worsen thyroid inflammation in some individuals. That's one reason I encourage people to be cautious about taking iodine supplements simply because they read online that they're good for the thyroid. Nutrients work best in balance. Too little can be a problem, too much can be a problem. The goal is adequate, not excessive. Before we move on, I also want to mention one nutrient that often gets overlooked in thyroid conversations. Protein. Protein isn't a thyroid supplement, but it's essential for maintaining muscle, supporting metabolism, repairing tissue, producing enzymes and hormones, and promoting healthy aging. As women enter midlife, preserving muscle becomes one of the most important things we can do for our lifelong health. Adequate protein intake combined with resistance training supports healthy aging regardless of whether you have thyroid disease or not. And that's one reason I spend so much time talking about muscle on this podcast, because muscle is one of the greatest predictors of health span. Bringing it all back together. When you step back and look at all of these nutrients together, one thing becomes clear. Your thyroid doesn't function in isolation. It depends on adequate nutrients, healthy immune function, good sleep, stress recovery, movement, and overall metabolic health. That's another reason I encourage women to think beyond simply asking, what supplement should I take? A much better question is what does my body need to function well? Sometimes the answer is medication. Sometimes it's improving nutrition. Sometimes it's treating an underlying medical condition. Sometimes it's prioritizing sleep. Most often it's a combination of several small changes working together. And that's exactly how healthspan works, not through quick fixes, but through consistent habits that support your body over time.

Healthspan Thinking And Next Steps

Before we finish today, I want to leave you with one thought. When most people think about the thyroid, they think about one diagnosis, hypothyroidism, hyperthyroidism, Hashimoto's, maybe a thyroid medication. But what I hope today's conversation has shown you is that your thyroid is part of something much bigger. It's one player on an incredibly well-designed team. And it's just like any other successful team, every player influences the other. Your thyroid communicates with your brain. Your thyroid communicates with your ovaries. Your thyroid responds to signals from your immune system. It depends on adequate nutrition. It affects your muscles. It influences your metabolism. It contributes to how efficiently your body uses energy. Everything is connected. That's why I believe one of the biggest mistakes we make in healthcare is assuming every symptom has one cause and one solution. Sometimes that's true. Many times it is not. A woman may come into my office because she's tired. She assumes it's her thyroid. Instead, we discover she has severe iron deficiency anemia after years of heavy menstrual bleeding. Another woman believes menopause is causing all of her symptoms. Instead, she develops Hashimoto's thyroiditis. Someone else thinks it's their thyroid, but when they're really experiencing is a culmination of poor sleep, chronic stress, low muscle, and inadequate protein intake. Three women, three very different stories, many of them with the same symptoms. That's why clinical reasoning matters. Here's why medicine is both a science and an art. As a clinician, we gather clues. We ask questions, we look for patterns, we rule possibilities in, and we rule possibilities out. Most importantly, we treat the person, not just the lab result. And I want you to think about your own health the same way. Not by trying to diagnose yourself, but by becoming curious. Instead of asking what's wrong with me, try asking what might my body be trying to tell me. That simple shift changes everything. It moves you away from fear and towards understanding, away from chasing symptoms, towards connecting the dots. And that's to me is what healthspan really is about. It's creating the healthiest, strongest, most vibrant version of yourself so you can fully enjoy the years you're given. It's protecting your brain, your muscles, your metabolism, your mobility, your independence, your energy, your purpose. Because adding years to your life is wonderful, but adding life to your years, that's the goal. And sometimes that journey begins by paying attention to the quiet whispers your body has been telling you all along.
